Well, I said it was troublesome with healing ... it migrated: The ring slowly crept toward the surface of the skin until it was about ready to pop out. When only 2mm or so of skin held the ring in place, I removed it.
I left it for about 2 months, and got it pierced again.
This picture was taken about four weeks after piercing, the red mark from the previous pierce can been seen just below the ring.
I have kept it covered whenever I wear a collar and tie this time, and support the ring underneath with a piece of surgical dressing. I hope that by healing the piercing with the holes as perpendicular to the skin as possible, the risk of migration may be reduced. This has tended to make the ring 'stick out', adding small weights once the healing process is complete should sort this out.

Addendum:
About a year further down the line, it's still in place, but I think it has healed up too shallow, and as a result of this, I think it is destined to slowly grow out.
Addendum 2:
Just over two years it lasted.
Don't know if I'll try it a third time.
